Post-Acute Rehabilitation

Providence Care Center offers a state-of-the-art, Medicare Certified, Joint Commission Accredited rehabilitation program designed for those recovering from acute situations such as cardiac conditions or stroke, fractures, and hip or knee replacements.  Our team of specialists provides the personalized physical, speech, occupational and aquatic therapies as well as other services needed to maximize performance, achieve optimal independence and get patients back to a normal lifestyle.

Memory Care

The Commons of Providence offers a memory care program in a secured neighborhood setting, comprised of 36 private suites. Memory care residents have exclusive use of beautifully appointed private dining, living and activity areas, and receive more intensive personal care and closer supervision by staff specialists 24-hours a day.

Skilled Nursing

Providence Care Center’s beautiful 138 bed skilled nursing area features fully furnished private and shared suites that include cable and flat screen TV. The Center’s dedicated and professional staff monitors each resident’s medical condition, and provides services through an individualized care plan. Our commitment is to render care in an atmosphere where dignity and compassion are held in the highest regard.

Providence Care Center is accredited by The Joint Commission and has earned the Gold Seal of Approval™ by demonstrating compliance with national standards for health care quality and safety in long-term care including infection prevention and control, leadership, performance improvement and medication management. Accreditation by The Joint Commission is a voluntary process that is above and beyond what is required by federal and state mandates, and demonstrates Providence’s commitment to the highest level of care for its residents.
